When admitted in a hospital.

It must be accepted that the people of India have not full faith in God. Even in hands of an expert doctor, they still pray before God that their relative, a family member or they themselves may come out of the hospital hale and healthy and fully treated. The people visiting them would not be encouraging them and they shall be telling those stories where the ill or the diseased people died because of wrong treatment or otherwise.

The period in hospital is full of tension, full of worries and pressures and instead of improving, there are chances that the ailing person may get more demoralization till he is in hospital. It would be better if we do not allow our relatives, others who want to see the ailing man or they have come as a customary visit and they must be kept out of the hospital. These people would not give encouragement to the ailing person, rather they shall be demoralizing the ailing person.

As far as we, the people of India are concerned, we are poor people and we know treatment has become very costly. Most of the expert doctors are not in government hospitals, but they have opened their own clinics and here charges are on the hire side and in government hospitals, most of the facilities are not available. That is also an additional tension and pressure on our mind. We are having an eye on the doctors and nurses attending our patient and when they are going out we run after them just to find out the latest position of the patient and his future. Each word of doctor shall be taken in mind and it can give us hope and pleasure and it can give us more tension. So as long as our patient is in hospital, we are not free and without pressure and if we ourselves are in hospital, then we are looking at the faces of doctors, at the faces of nurses and at the faces of our relatives attending us and if they have a smile, we too smile and if they are sad, we also go deep in sadness and frustration and we have, actually no hope that we shall survive. We know that in India we take the patient to the hospital only when we are not having any hope.

So these pressures, these tensions and these worries are but natural and we cannot avoid these worries and tensions and therefore, we should bear all this and take these things as a course of life. We shall have to develop a strong will to face all these contingencies of life because these are usual and customary in our life. We must be preparing ourselves for these contingencies of life and at least the people of India and people in other developing countries must be hard to face such types of contingencies in life.

It is very easy to find a doctor in the United States. Hospitals: You can find list of doctors in every hospital. Visit a nearby hospital and get the list from which you can get to know about individual doctor’s specialization as well as their contact numbers. Sometimes the list also mention when the doctor will be available in the hospital. If there are names of doctors in the list and no phone numbers, check the yellow pages under doctors and hospitals, you can get their phone numbers.
Health Insurance: Health insurance companies suggest a list of doctors to the insured and that will be in the policies. Some insurers provide lists of physicians whose services are covered by the plans. If you get treatment under them the expenses will be paid by the insurance companies.
Doctors’ associations: There are medical societies from where you can find names of doctors and their contact numbers and addresses. The American Medical Association (AMA) has list of doctors displayed at its website. Most specialties have their own organizations, such as the American Academy of Pediatrics. You can find a specialist also from the list of organizations.
Know from people close to you: You can ask friends, neighbors and colleagues for a doctor. Some of them must have been treated by a good doctor and that can be of better help to you. In that case, reliability on the doctor will be more.
Walk-in clinics: In all cities and towns of the US, you can find walk-in clinics. Without appointment, you can go and meet the doctor or dentist. You can find many expert doctors in walk-in clinics.
Directory: You can find the list of doctors in your city from the yellow pages. See the list under physicians and get their numbers and addresses.
Internet: World Wide Web is the best source to get the list of doctors. Most of the hospitals have their websites and list of doctors in the internet. Also, doctors are now putting their names and addresses in the internet as advertisement of their services.

I recently watched a special on TV about hospitals and their aggressive debt collection tactics. It was unbelievable – hospitals were trying to collect on 20 – 30 year old debts for dead people by obtaining leans and using corrupt lawyers, politicians, and lobbyists to make it so they can basically get away with theft from our uninsured and under-insured – which is now over 50% of americans.
Hospitals were aggressively using body attachments where they basically have someone arrested for an overdue bill. This is the most aggressive form of collection tactics and harassment. Most collection agencies refuse to resort to these tactics. It is debtors prison revisited – debtors prison was made illegal in the U.S. in the 19th century. It is basically a method of extreme harassment to coerce payment from the debtor. Typically it results in lost wages for the debtor, extreme hardship as many have children, etc…
Many of these hospitals have also resorted to zombie debt collecting – trying to collect on debts that are un-collectable (debtor passed away, over 10-30 years old and non-enforcable, etc…) Several hospitals have been using these tactics to threaten and coerce surviving spouses into taking on old debt. Some have even been caught trying to collect on debts expunged in bankruptcy.
The worst part is how these hospitals and doctors determine what the actual amount of the debt is. Medicare and insurance have set prices that they must agree to. But the uninsured are charged a lot more – a whole lot more. At Presbyterian Hospital in Charlotte NC the uninsured are charged many times over what the insured are assessed – 300 – 400% more and even more. With a little research it was found that this hospital, like many others, was undoubtedly taking advantage of the uninsured. If a patient tries to see how the bill came to be – itemized – it is almost in foreign code. They don’t want you to see the $100 toilet paper, 1000% markup on medicines and supplies, etc… It is a license to literally steal from the uninsured – they can make up whatever numbers they want for charges and collect very aggressively on them. This needs to be changed – these hospitals and doctors need to be policed. It used to be that the oath a doctor had to take to become a doctor was to basically help people – it has even been changed to favor the doctor and his earnings.
The hospitals say that they are having a hard time covering costs and such – right, thats why the CEO and other board members make millions of dollars for little work. The problem we have here is far greater than we can see. The healthcare industry is very unstable, prices are soaring, medicare and insurance are also increasing at rates that are causing coverages to be lowered or even dropped all together. What the U.S. needs is for the government to take back control. Limit doctor pay, hospital charges, billing practices, etc… Increase taxes on the wealthy – stop giving them so many tax breaks and shelters. The average american (low to middle class) pays around 30 – 35% in taxes, while the affluent pay only 1% or less on average. Hospitals and doctors are playing a game of legalized theft – enough is enough. Lets take back our health care system and return it to what it once was and should be – to provide low cost, fair healthcare regardless of race, financial background, etc…

Delhi is an attraction for numerous things. People visit here for varied reasons. Some come to Delhi for travel and some sections of the people come for treatment. When they lose hope and their local doctors put their hands up, they come here to cure deadly diseases. All India Institute of Medical Sciences, Apollo Hospitals are very famous and well-known hospitals in Delhi and patients come here from every nook and corner for treatment. Those big and famous hospitals of Delhi have been a ray of hope for the patient and their relatives. Once they step into AIIMS or Apollo, they feel that 70% of their pain has been cured.

No doubt, that AIIMS and Apollo hospital is a buzz house of talented and well-experienced doctors. Patients find their doctors demi God who can do miracle. If after a long treatment they are not cured, then all their hopes and aspirations are shattered down. Hospital is an institution of health care. Hospitals in Delhi provide all the basic amenities to their patients and also provide a helping hand to the relatives. The doctors are lenient and always try their level best to cure their patients. Doctors and staffs strive to provide comprehensive, high quality tertiary care services including specialty and super-specialty services. They never discriminate on the basis of caste and creed. People find the hospitals in Delhi very advanced.

Speaking about the top most hospital in Delhi i.e., AIIMS, we can say that it is an institution which works only for the betterment of their patients. They try their level best to provide the best treatment to the ailing. Doctors always get to experience new diseases and they research on that and find solutions to fight against those diseases. All modern equipments can be found here. Departments like Neurology, Neurosurgery, Oncology, Ophthalmology, Rheumatology, Endocrinology, ENT, Pediatrics, Pediatric Surgery, Pediatric Neurology, Urology, Nephrology, Dermatology, Dentistry, Plastic Surgery, Gynecology, Pulmonology, Psychiatry, General Medicine & General Surgery can be found in all the famous hospitals of Delhi. There are sufficient numbers of patients and doctors to take care of the patients. The doctors and staffs of the hospitals in Delhi work hard to stand up to the expectation of the people.

Honestly speaking, today the internal politics of the doctors and the staffs has polluted the environment of the hospitals. Doctors are playing with the life of the patients to satisfy their personal benefits. They are taking their noble profession too lightly. The bad activities of the doctors are being flashed on the newspaper very often. At times, the doctors become so negligible that they leave small equipments on the operating body and they escape unnoticed. Hope the time comes very soon when the doctors will truly give value to their profession and give their heart and soul maximum in treating their patient honestly and sincerely.

Finding a suitable hospital in a foreign country is a matter of serious concern for a great many. There is no universally acceptable guide to measure the quality of hospitals like that of hotels and other services. A prospective patient should understand the technology needed for the treatment along with the highly trained doctors and specialists. They should also pay attention to the level of team work and nursing services in the hospital.
The Triad of Health Care

After careful analysis, it has been postulated that the three pillars of modern medical treatment are:

(i) modern technology

(ii) trained doctor/specialists

(iii) team work and nursing care
They are intricately related to each other and if any of these are weak then the level of care at a hospital cannot reach the highest level.
Modern Technology :

Technology is at the core of everything, ranging from laboratory tests to specific treatment methods. There are many types of HIV test, ranging from $1 – $70. All tests differ significantly in terms of sensitivity and specificity.
The same concept applies to CT scans. Old models are capable of 2-4 slice CT images, but newer machines allow up to 16 slices. While the newer models may cost more, the high resolution images allow radiologists and physicians to make a more accurate interpretation of the data. The 16-slice CT also supports 3-D imagery, which makes virtual surgery possible with the aide of computer software analysis, among other benefits. This new technology enables doctors to come up with a clearer diagnosis, something almost impossible with the older machines.
A coronary by-pass operation (CABG) that uses a conventional heart-lung machine is cheaper compared to a new off-pump or beating heart technique. The off-pump operation has lower risk of failure, less complications, less blood transfusion, and shorter duration of hospitalization. Generally, conventional CABG uses veins from the leg or arm as the by-pass graft, but may not last more than ten years.
Do all patients know or understand the significance and advantages of these new technologies? Perhaps not, Even many physicians do not know all of them. This is because of the fact that medical science is developing very fast and newer technologies are added everyday. Many doctors who do not engage in any form of continuing education, read medical journals, or attend seminars have difficulty upgrading their knowledge. Some specialist doctors who only study their own area may not know what is happening in other areas of medicine.
Trained doctors and specialists :

Doctors and specialists are essential when it comes to using modern technology. One of the main factors for a doctor to make a good decision about patient management is to have the latest knowledge of medical science which is developing at a tremendous pace. It is a tall order for a practicing doctor to study medical journals, attend seminars and conferences, and then undergo training with the new technologies.
Precisely for this very reason, many developed and newly emerging countries are placing much emphasis on Continuing Medical Education (CME). The CME is an essential ingredient to keep doctors abreast with new development. The doctors not only learn about the areas they work with, but also learn and understand other areas of medicine that they do not practice.
Specialist doctors also sub-specialize in various areas. For example, a cardiologist may subspecialize only in Interventional Cardiology i.e. Angiogram and Angioplasty. Other cardiologists may sub-specialize in Arrhythmias and their management. An orthopedic surgeon may sub-specialize in Arthoplasty and Joint surgery. Others may do only spinal surgery. So it is more useful to know the problem in detail before selecting a particular doctor for treatment.
Sometimes it is very useful to visit an Internal Medicine doctor before visiting a specialist. While specialists know the treatment for their particular field very well, they may overlook problems in other parts of the body. For example, a patient with chronic back pain may visit an orthopedic surgeon. However, it is quite possible that the cause of the problem may originate from some internal organ like the urinary system, colon, even stress or major depression.
In such cases, the Internist will review all systems at the beginning and then refer to the specialists as needed. I have seen several patients receiving treatment for an illness that they do no suffer from, because they had gone straight to a specialist before consulting an Internist.
Choosing a right doctor in a foreign hospital is a serious matter; not all of them are the same. Usually experienced doctors are very busy with many patients, mostly locals who know them well. It isn’t very easy to get an appointment with them from abroad. So a walk-in patient may end up with a not-so-popular doctor.
Team work and nursing care :

This is probably the most delicate part of high quality patient care. In many instances, it is not one doctor who does everything regarding treatment. This is especially true if the patient with complicated illnesses is admitted into a hospital. A neurosurgeon may do a brain surgery and then leave it up to the ICU doctors to take care of the post-operative care.
She/he also needs good cooperation from other doctors concerning proper management of blood pressure, nutrition, electrolyte imbalance, infection control, etc. It is also necessary to have a physical therapy doctor who helps early ambulation and rehabilitation after brain surgery. Nursing care is vital throughout the hospital treatment. They provide physical and mental support to the patients’ well-being.
One should keep in mind that because of the very dynamics of medical treatment, the quality of care in a hospital changes over time. So one should keep the eyes and ears open before selecting the right hospital.
